Mass Chest Wall (Differential Diagnosis Mastitis)
1.5-month-old girl with a large mass of the left lateral and upper chest wall. At the top of the swelling there is a discharge of a yellowish liquid; adjacent to the normal chest skin a bluish and reddish area is visible.
The diagnosis is chronic lymphadenitis with abscess following a tuberculosis vaccination at the left upper arm in the neonatal period.
In this type of lymphadenitis there is the danger of chronic fistulization without or after abscess incision. The local findings are a mitigated form of primary tuberculous infection (= site of vaccination) with distinct involvement of the draining regional lymph node. Although today the tuberculosis vaccination is only rarely performed, it may be necessary in patients at risk, even at the risk of such a complication.
