Moistening Perineal Orifice (Differential Diagnosis Voiding Disorder)
9-month-old infant with a moistening orifice above the anus; the latter is plugged with a piece of gauze.
Loss of urine can be seen in this case due to an incomplete type of urethral duplication with loss of urine by an additional perianal orifice at an ectopic site. On endoscopy, an accessory opening at the level of the colliculus as starting-point of the additional urethra has been observed.
This variation of urethral duplication is rare; more often, fistulas of the penis are observed which have no connection to the urethra, no urine transport, and a single penile opening. In the presented case, excision was necessary due to the dripping of urine and the danger of an ascending infection.
