Very Large Defect (Congenital Diaphragmatic Hernia)
View from the opened abdominal on the left thoracic cavity in a 1-day-old girl. Within the defect the base of the lower lobe of the left lung is visible. The border of the defect is missing dorsally, is very small laterally, and runs into an intact anterior part of the diaphragm. Notice the missing fixation of the reduced colon at the left posterior abdominal wall, which is an example of the regularly present malrotation in congenital diaphragmatic hernia.
It is a very large defect of the diaphragm of the type of an open pleuroperitoneal formamen. This Bochdalek's hernia has no hernia sac.
